Patient Guide
The following are some important guidelines, procedures and policies that you should know in order to help make your hospital stay more convenient and pleasant. Please review them and don’t hesitate to ask our Nurse-on-duty if you have any questions. Additionally, you are also welcome to visit our Information Department situated on the ground floor of the Main Building for any queries.
Admission
Our Admitting Staff will provide you with information on room rates, bed availability, and will arrange the details of your admission. Information requested from you at this time is vital and is intended to make your medical record accurate and complete.
Company /HMO Subscribers
A Letter of Authorization (LOA) and/or a currently valid ID card is required upon admission, especially for elective cases. Said LOA must be submitted to our HMO/Company Clerk at the Business Office upon admission.
Philhealth
Philhealth forms can be obtained from the Admitting Section during office hours, located at the 1st Floor Main Bldg. Any inquiries regarding Philhealth requirements can be made at the Admitting Section shortly after admission so you will have ample time to prepare necessary documentation.
If you submit your complete Philhealth papers before discharge, you can take advantage of your Philhealth benefits. Otherwise, failure to present the required documentation within a specified period following admission would mean that the patient will be responsible for paying hospitalization deposit as well as all hospital bills which are accountable to him or her.
Consent Form
Our Admitting Staff will let you fill up a consent form for admission.
Senior Citizens Discount
All out-patient and in-patient treated in this hospital can avail of the 20% Senior Citizens discount only upon presentation of a duly signed OSCA I.D.

Patients’ Responsibility
- Provide a complete and correct medical history (Responsibilidad mong magbigay ng tamang kasaysayan ng iyong paggagamot at ng iyong pamilya. Kabilang dito ang anumang problema, komplikasyon o hindi mabuting reaksyon iyong naranasan)
- It is your responsibilty to ask specific questions and information about problems that arises during the medication process. (Responsibilidad mong magtanong ng mga impormasyon tungkol sa mga problemang maaring lumabas o makita habang kayo ay nag-papagamot)
- Participate in healthcare decisions with the advice of your doctor(s) and follow recommendations and advice of your doctor(s) (Responsibilidad mong sundin ang mga plano sa pangangalaga sa iyong kalusugan ng mga doktor)
- Inform your doctor or nurse about any problems encountered during your medical treatment. (Responsibilidad mong sabihin sa doktor o nars ang mga problema na meron ka tungkol sa iyong pag-gagamot) •Be considerate of the right of patients, DDVMH Staff, and property (Responsibilidad mong i-kunsidera ang mga karapatan ng ibang pasyente, ng mga empleyado at ang mga kagamitang pag-aari ng ospital)
- Prompt payment of hospital bills including partial payments (Responsibilidad mong magbayad sa takdang oras)
- It is your responsibility to take care of your valuables. The hospital is not liable to any loss or damage. (Responsibilidad mong huwag magdala ng mga mahahalagang bagay gaya ng alahas, mamahaling gamit at iba pa maliban sa mga kakailanganin lamang sa iyong paggagamot. Ang hospital ay hindi responsable sa mga kagamitang nasa iyong pangangalaga)
- Follow DDVMH hospital’s rules and regulations (Responsibilidad mong sundin ang mga regulasyon ng ospital tungkol sa iyong kalusugan sa panahong ikaw ay nasa hospital)
Visiting Hours
- Visiting Hours will ONLY be allowed from 9:00 am to 12:00nn and between 5:00pm to 9:00pm for Ward, Private and Suite Rooms. The same time will also be allowed for viewing of patients in NICU (Nursery). While no visitors are allowed to enter in the ICU area.
- Only 2 visitors at a time will be allowed to enter with the maximum of 1 hour. (This is to give time for the patient to rest and minimize exposure to possible disease carriers among visitors)
- Sick visitors will not be allowed to enter.
- Children less than 10 years old are not allowed to visit except in extreme of cases like end-of-life conditions. They are, however, required to wear mask and must visit for shortest possible time. (< 5 mins).
- All visitors must wear surgical mask at all times while inside the hospital premises.
Watchers
- The hospital only allows 2 watchers at a time for each patient.
- Exchange of watchers may be done at any time of the day.
- No sick watchers will be allowed to stay
- Minors (<18 years old) are not allowed to be watchers
Deposit
No deposit is required on Admission but a progress Billing Statement shall be forwarded to you every day update your current account.
Billing during Confinement
Please take note that it is a Hospital Policy to collect a partial bill payment for in-patients whose statement has reached P10,000.00 or for every two (2) days of confinement, whichever comes first.
If accumulated bills are not settled, you will be advised to transfer to a lower room category or to a government hospital. When accumulated bills reached a certain predetermined amount, and no advance is placed within a specific time period, diagnostic procedures and other services ordered subsequently will have to be paid in cash. You can request for a partial or final statement of account print out at the Billing Section.
Billing Office Hours:
Monday to Saturday – 8:00 AM to 6:00 PM
Sunday – 8:00 AM to 5:00 PM
The hospital has provided a monitor at each Nursing Station for you to view your running billing statements. Discharge order after 6:00 PM shall be discharged the following day. Please transact with authorized personnel only and settle your accounts at the Cashier Section. Always ask for an official receipt. Any damages / breakages in your room shall be added into your billing statement.
Billing of Elective (Non-Emergency) Operation
All in-patient must secure an OR clearance from the Business Office prior to a scheduled elective procedure by placing a 40% advanced payment on the ‘estimated’ operation cost package. Consent forms must be submitted.
A day after the operation or upon full recovery (depending on which came first), the patient or his immediate family must settle the remaining balance of the operation. If the patient is unable to immediately pay the balance, it is management’s discretion to transfer the patient to a ward room or to the nearest government hospital.
Room Transfer
If patient transfers from WARD to PRIVATE (vice versa) or ICU to WARD/PRIVATE, a partial payment of 50% percent of the running bill is needed, to prevent accumulation of bills. But if patient transfers from WARD/ PRIVATE to ICU, no settlement of partial payment is required.
Checks
Personal checks are not accepted for payments. Managers checks are allowed.
Credit Cards/Debit Cards
The hospital accepts VISA / Mastercard / AMEX / Medilink / BPI Credit Card as payment for in-patient bills. We also accept payments via Gcash, and BDO Online Banking.
Accredited HMOs
The hospital honors several Health Maintenance Organizations (HMOs). For further details, you may inquire at the Business Office located at 3rd floor of the main building or you may also proceed in the HMO catering area situated at the ground floor of the same building. Check-Out and Discharge
During discharge, your doctors authorized order shall be written on your patient’s chart. Please coordinate with the Nursing Station before going to the Billing Section to secure your Final Statement of Account (SOA). Prior to discharge, please proceed at the Satellite Pharmacy located at the 3rd floor of the main building for return of un-consumed medicines. Returned Medicines are accepted before the actual discharge of the patient.
Nursing Station will advise you to go to Billing Services for the following procedures:
- Issuance of Final Statement of Account and Professional Fees
- Submission of Senior Citizen ID
- Submission of LOA for Company-Sponsored and HMO accounts
- Filling and Submission of Phil Health Form
Discharge Against Advice
Should you wish to leave the hospital against your Attending Physician’s advice, you will be required to sign a “Release from Responsibility” Form, also called the HAMA (Home Against Medical Advice) form.
Discharge Cut-off Time
First Cut-Off: 12:00 Noon
Second Cut-Off: 6:00 PM
These cut-off times will be strictly followed to ensure that all necessary billing, medical clearances, and other discharge-related procedures are completed in a timely manner.
-Patients cleared between 12:00 NN – 6:00 PM should be discharge until 6 PM.
-Patients discharged after 12:00 NN will be charged an additional half-day room rate. Patients discharge after 6:00 PM will be charged an additional full day room rate.
PhilHealth Refund
If you are not able to submit the duly accomplished forms and complete the required documents on your discharge date, these documents maybe submitted to our PHIC Officer-On-Duty for processing.
You may file your claim directly with Philhealth within the latter’s prescriptive period of sixty (60) days from your discharge date. However, we request that you submit the documents within forty (40) days from your discharge date to our PHIC Officer-On-Duty for the preparation of the transmittal report to Philhealth.
Your claim can be refunded to you directly by Philhealth provided the original and/or certified true copies of statement of accounts and official receipts as proofs of payment are submitted to the hospital together with the duly accomplished Philhealth forms I & Il.
If your hospital bill is sponsored by a company or HMO accredited by the hospital, a letter from said company or HMO stating that Philhealth benefit shall accrue to the member is required, otherwise the refund will accrue to the company or HMO.
Ambulance Service
DDVMH offers ambulance services with varying rates determined according to distance from the hospital and waiting time.
Pharmacy
Hospital Pharmacy is open 24 hours to ensure quality of service in terms of dispensing and compounding various pharmaceutical products. A Satellite Pharmacy is also available at the third floor beside the Central Supply Room.
Security Services
All bags, packages and equipment brought in and out of the premises are subject to inspection and shall be issued with gate passes for security reasons.
Identification
Each patient is provided with an ID wrist tag containing vital Information and should be worn at all times while admitted. Please do not attempt to remove the wrist tag until after you have been discharged from the hospital.
Inquiries & Suggestions
Kindly channel any inquiry or suggestion through the Head Nurse or Nursing Supervisor-on-duty or any Management Staff. Please fill in the Patient Evaluation Form before leaving the hospital. Patient Evaluation Forms are available at the Nurses’ Station and at the Information Section.
Money and Valuables
THE HOSPITAL IS NOT RESPONSIBLE FOR ANY LOST CASH AND VALUABLES. It is therefore advised that you should not leave your things unattended, even in your vehicles.
